- For each patient, an individual care plan is created which may include the following services:Peritoneal dialysis, Hemodialysis (in-center and home-based), Evaluation and exploration of treatment for patients with end-stage renal disease, including transplant, self-care, home training and peritoneal dialysis, Ongoing care and support for home patients, Educational programs designed to improve care and increase understanding of patients with renal disease and their families, Ready availability of integrated multi-disciplinary specialty consultation, Nutrition consultation and support, Specialized pharmacy and nursing services, State-of-the-art medical record and ordering systems.Choice of admission to Meriter, UW Hospital and Clinics or St. Marys Hospital, depending on insurance and the primary nephrologist for each patient.Know Your OptionsExplore your options for dialysis treatment if you have chronic or end-stage renal disease (ESRD). Wisconsin Dialysis offers an Option class one to two times per month for those interested in learning more about hemodialysis, home dialysis programs, nutrition, dialysis access options and maintaining the kidney function you have.
